A Call Out To Stakeholders - Share Your Software Application Development Experiences

The recent blog posts I added, centered around Agile development methodologies, generated a good deal of heated debate, largely in defense of Agile methodologies. The two posts are here; the first one here and the follow on one, with more detail about the source of the debate, here. The second link, in particular points to a really good thread, which touches on the main point of this post, too many software projects are still failing and I really truly believe this is a solvable situation.
So this post is a call out, to stakeholders, to the ones paying the money for what our development community provides. What have your experiences been with software application development both good and bad and were you kept informed at all times what platform or methodologies were being used and why? We can and have to improve the success rate for software projects, software will drive our success into the future at all levels of our daily lives.
